To meet the requirements for evaluation, all submitted materials must demonstrate academic integrity and originality. No more than 30% of the submission may consist of direct quotations or closely paraphrased content from external sources, ensuring that the work reflects the student’s independent effort and understanding. All documents must be submitted as file attachments in approved formats, including .docx, .pdf, or .ppt. Cloud-based submissions, such as Google Docs or OneDrive links, are not acceptable and may result in rejection of the submission.
In addition, students are expected to strictly adhere to the provided grading rubric. The rubric outlines specific criteria against which the submission will be evaluated, and failure to align with these expectations may negatively affect the evaluation outcome. Following these guidelines ensures a smooth review process and confirms compliance with academic and program standards.
The Clinical Practice Experience (CPE) Record must be submitted as a separate document and should not be embedded within the e-portfolio. This document must be fully completed and signed to verify the authenticity and completion of all required activities. The CPE Record serves as official documentation of clinical engagement across both Phase 1 and Phase 2 of the course.
To be considered valid and complete, the CPE Record must include the following elements:
The student’s full legal name (first and last)
The official submission date
The name of the assigned course instructor
Documented completion dates for each listed activity
A typed signature confirming that all required activities for Phase 1 and Phase 2 have been successfully completed
Including all required information ensures the record is verifiable, accurate, and compliant with course expectations.
The e-portfolio must contain eleven distinct deliverables, representing work completed during both Phase 1 and Phase 2 of the Clinical Practice Experience. Each deliverable must be uploaded directly into the e-portfolio platform and accompanied by a functional “Share” link to allow evaluators full access to the materials. Incomplete uploads or broken links may result in the submission being marked as not evident.
The required deliverables and their descriptions are summarized in the table below:
| Deliverable | Description |
|---|---|
| CPE Schedule Table | A detailed timeline outlining all planned and completed clinical activities. |
| Advocacy Plan Summary | A 200–300 word overview identifying the selected Social Determinants of Health (SDOH) and their impact. |
| SMART Goal | A structured goal statement addressing Specific, Measurable, Achievable, Realistic, and Timely components, with one sentence per element. |
| Policymaker List | A list of relevant policymakers, including names and accurate contact information. |
| Phase 1 GoReact Screenshots | One screenshot of the student’s video submission and two screenshots showing feedback provided to peers. |
| Phase 1 Reflection | A reflective narrative summarizing learning experiences and insights gained during Phase 1. |
| Team-Building Strategies | A list of seven strategies implemented to strengthen collaboration and teamwork. |
| Interprofessional Stakeholders | Identification of five key stakeholders representing different professional disciplines. |
| Meeting Agenda | Four clearly defined agenda items prepared for the first advocacy action team meeting. |
| Phase 2 GoReact Screenshots | One screenshot of the Phase 2 video and two screenshots of peer feedback provided. |
| Phase 2 Video Reflection | A written reflection discussing outcomes, challenges, and professional growth during Phase 2. |
All deliverables must be complete, clearly labeled, and properly linked to ensure accessibility during evaluation.
Submissions are evaluated using a competency-based rubric that focuses on two primary areas: the CPE Record and the E-Portfolio Deliverables. Each area is assessed using a binary scoring system—“Not Evident” or “Competent.” This approach emphasizes completeness, accuracy, and adherence to submission requirements rather than partial credit.
The evaluation criteria are outlined in the table below:
| Requirement | Not Evident Description | Competent Description |
|---|---|---|
| CPE Record | The record is missing, incomplete, undated, or lacks a required signature. | The record is fully completed, dated, signed, and submitted as a separate attachment. |
| E-Portfolio | One or more deliverables are missing, incomplete, or linked incorrectly. | All eleven deliverables are complete, properly uploaded, and accessible through functional links. |
Achieving competency in both categories is essential for the submission to be accepted and formally evaluated.
