During medication retrieval, the nurse used an override function on an automated dispensing cabinet and entered “VE” while searching for the medication. Vecuronium, a neuromuscular blocking medication, was selected instead of the prescribed midazolam. Vecuronium causes skeletal muscle paralysis and can prevent spontaneous breathing.
The medication was administered, and the patient was transported for imaging. The patient subsequently became unresponsive and experienced cardiac arrest and severe brain injury before dying.
