Dementia generally involves a gradual decline in cognitive function that interferes with daily activities, whereas delirium is an acute disturbance in attention and awareness that develops over hours to days and commonly fluctuates. Delirium is often caused by an underlying medical problem and should be treated as a potentially urgent condition.
Brain imaging is performed to determine whether intracranial hemorrhage is present and to evaluate the brain before reperfusion treatment. Thrombolytic therapy is intended for eligible patients with acute ischemic stroke and can be dangerous when intracranial bleeding is present.
Metformin remains an important first-line medication for many adults with type 2 diabetes, but current treatment is increasingly individualized. GLP-1 receptor agonists and SGLT2 inhibitors may be prioritized for patients with cardiovascular disease, heart failure, chronic kidney disease, obesity, or other specific indications.
Pyridoxine, or vitamin B6, helps prevent isoniazid-associated peripheral neuropathy in patients who have risk factors for this complication.
A commonly cited therapeutic serum concentration is approximately 5–15 mcg/mL. Because theophylline has a narrow therapeutic index, concentrations above the therapeutic range can cause serious toxicity, including arrhythmias and seizures.
The cardinal motor features are bradykinesia, resting tremor, and rigidity. Postural instability can develop as the disease progresses. Shuffling gait, freezing, and reduced arm swing are also common clinical findings.
Long-term oxygen therapy benefits selected patients with severe chronic resting hypoxemia who meet established clinical criteria. It should not be prescribed solely because a patient has COPD or experiences occasional shortness of breath.
Rapid growth, hoarseness, cervical lymphadenopathy, fixation, and suspicious ultrasound characteristics can indicate a higher risk of malignancy and warrant further evaluation.
DKA is characterized by ketosis and metabolic acidosis and commonly occurs in people with type 1 diabetes, although it can occur in type 2 diabetes as well. HHS usually occurs in type 2 diabetes and is characterized by severe hyperglycemia, profound dehydration, increased serum osmolality, and little or no significant ketosis.
Stopping tobacco use reduces the risk of cardiovascular disease, stroke, COPD progression, lung cancer, and other tobacco-related diseases. Smoking cessation also improves long-term health and reduces premature mortality.
