Have you experienced numbness, tingling, or muscle weakness?
Have you noticed changes in your gait or balance?
How does the pain affect work, sleep, exercise, and daily activities?
What treatments or medications have already been attempted?
Have you experienced similar episodes in the past?
Do you have other chronic medical conditions?
Have you experienced bowel or bladder changes or numbness around the groin or saddle region?
These questions help distinguish lumbar radiculopathy from other potential causes of lower back and leg pain, including spinal stenosis, peripheral neuropathy, vascular disease, musculoskeletal disorders, infection, and malignancy.
Lumbar radiculopathy occurs when a lumbar spinal nerve root becomes irritated or compressed. It commonly produces radiating pain into the buttock or leg and may also cause numbness, tingling, sensory changes, or muscle weakness.
In this case, the distribution of pain toward the dorsum of the foot raises concern for L5 nerve root involvement. However, the patient’s substantial improvement with forward flexion and worsening symptoms with walking and lumbar extension also make lumbar spinal stenosis an important consideration.
Therefore, the clinical presentation is best approached as suspected lumbar radiculopathy with possible spinal stenosis rather than assuming a single structural cause before completing the neurological examination and diagnostic evaluation.
Several findings support possible L5 radiculopathy:
Radiating pain from the lower back into the lower extremity.
Pain extending toward the dorsum of the foot.
Burning and cramping sensations.
Reduced walking tolerance.
Increased symptoms with lumbar extension.
Improvement with forward flexion.
Altered gait and a flexed standing posture.

Deep tendon reflexes.
Signs of nerve tension or irritation.
Hip and lower-extremity function.

Electromyography (EMG) and nerve conduction studies may be useful when the diagnosis is uncertain or when it is necessary to differentiate radiculopathy from peripheral nerve disorders.
Treatment should be individualized according to symptom severity, neurological findings, functional limitations, and the underlying cause. Many patients improve with conservative treatment, particularly when there is no progressive neurological deficit or other serious pathology.
The primary goals are to reduce pain, maintain mobility, improve function, and prevent unnecessary disability.
Medication may be used as part of a broader treatment plan for pain control. Depending on the patient’s medical history and contraindications, clinicians may consider medications such as nonsteroidal anti-inflammatory drugs (NSAIDs) or other appropriate analgesics.
Because this patient has hypertension, medication selection should take his cardiovascular and renal risk factors into account. NSAIDs can increase blood pressure and may affect kidney function or gastrointestinal health, particularly with prolonged use.
Medication should therefore be selected based on the patient’s individual risk profile rather than relying on a fixed regimen for every patient.
If symptoms remain severe despite conservative management, referral to a spine or pain specialist may be appropriate. Selected patients may be considered for additional interventions, including epidural steroid injections, particularly when radicular symptoms remain significant. Surgical consultation becomes more important when there is progressive neurological impairment, severe persistent symptoms associated with structural compression, or another clear surgical indication.
Non-pharmacological treatment plays an important role in recovery. Physical therapy can help improve mobility, strength, flexibility, posture, and functional capacity.
Appropriate interventions may include:
Individualized physical therapy.
Core and trunk strengthening.
Flexibility and mobility exercises.
Activity modification.
Postural and ergonomic education.
Gradual return to normal activities.
Low-impact physical activity as tolerated.
Weight management when appropriate.
Heat or cold therapy for short-term symptom relief.
Prolonged bed rest should generally be avoided because inactivity can contribute to deconditioning and reduced functional capacity. Patients should remain as active as their symptoms safely allow while gradually increasing activity.
Patient education is an important component of treatment. The patient should understand that improvement may occur gradually and that maintaining appropriate activity can support recovery.
Education should address proper body mechanics, posture, activity modification, medication safety, and adherence to physical therapy recommendations. The patient should also understand which symptoms require urgent medical attention.
Although this patient currently denies major red-flag symptoms, he should be instructed to seek prompt medical evaluation if new concerning symptoms develop.
These include:
New loss of bladder or bowel control.
Difficulty initiating urination.
Saddle or perineal numbness.
Progressive or severe lower-extremity weakness.
Rapidly worsening neurological symptoms.
Fever associated with severe back pain.
Unexplained weight loss.
Severe pain following significant trauma.
New bowel or bladder dysfunction, saddle anesthesia, or rapidly progressive neurological deficits can indicate cauda equina syndrome, a medical emergency requiring immediate evaluation.
Follow-up should focus on pain severity, neurological status, walking tolerance, functional ability, and response to treatment. Because the patient has already experienced declining walking capacity and a change in gait, his functional status should be monitored closely.

A focused neurological examination and appropriate follow-up are essential for confirming the diagnosis and guiding treatment.
