A 37-year-old female presenting with irregular, shortened menstrual cycles, fatigue, and thinning hair following the discontinuation of oral contraceptives is most likely experiencing Polycystic Ovary Syndrome (PCOS). Confirming this endocrine disorder requires a targeted diagnostic strategy—including total testosterone, thyroid-stimulating hormone (TSH), ferritin levels, and pelvic ultrasound imaging—to rule out secondary conditions such as endometriosis or uterine fibroids and to manage long-term metabolic risks.
Patient Presentation and Clinical History
Cindy Haas, a 37-year-old female high school teacher (Encounter #1524523, evaluated September 26, 2023), presented with a primary chief complaint of abnormal menstrual patterns lasting over a year. The patient noted that her menstrual cycles became noticeably lighter and shorter—lasting only two to three days and requiring just one to two pads daily—shortly after she discontinued hormonal birth control. She experiences mild lower abdominal cramping during menses but denies generalized pelvic pain, nausea, bowel changes, or urinary distress. Previous self-management strategies, including daily over-the-counter prenatal vitamins and acupuncture treatments, yielded no noticeable improvement.
The patient’s past medical history is free of chronic illness, major trauma, surgical interventions, or prior hospitalizations. She reports a known medication allergy to sulfa-based drugs, which causes a cutaneous rash, but reports no food or environmental intolerances. Her social history indicates a stable home life, married for 10 years, with no history of tobacco or recreational drug use and occasional alcohol consumption on weekends. Assessment and Differential Diagnosis
Primary Diagnosis: Polycystic Ovary Syndrome (PCOS)
PCOS represents the primary suspected diagnosis. The onset of oligomenorrhea following oral contraceptive cessation, combined with clinical signs of hyperandrogenism (hair thinning), systemic fatigue, and a strong familial background of Type 2 diabetes, aligns closely with Rotterdam diagnostic criteria for PCOS. This condition involves abnormal luteinizing hormone dynamics and elevated androgen production, leading to ovulatory disruption, subfertility, and long-term insulin resistance.
Differential Diagnosis: Endometriosis

A formal referral to an Obstetrics and Gynecology (OB/GYN) specialist is recommended for advanced endocrine management. A follow-up clinical evaluation is scheduled in 12 weeks to review diagnostic imaging, evaluate hormone panels, and adjust the therapeutic plan based on symptom resolution.
