The nursing assessment for Mr. Smith indicates that he desires assistance in managing both his stress and blood pressure. He prefers to avoid medication and is committed to making lifestyle changes. The nurse’s assessment includes educating Mr. Smith on stress management techniques, proper nutrition, and physical exercise to address his health concerns.
